What is AD&D Life?
This is purely in respect of Accidental Death (an unforeseen death emanating from accident) and Dismemberment. This should not be confused with Standard or Universal Life Insurance.

What is an Outpatient Surgery?
As per the benefit+ plans, an Outpatient Surgery is defined as a surgery that does not require an overnight stay and is performed by a doctor in either a hospital or ambulatory surgical center.

What is Temporary Total Disablement (TTD)?
This is a benefit for a disablement which entirely prevents the policy holder/employee from attending to their business or occupation.

What is Diagnostic Imaging?
When ordered by a Physician, these services include Radiology, Ultrasound, MRI, EKG, EEG, and so on.

For the hospital cash benefit, how is a “day” defined?
Days are defined in 24 hour increments.

For the Hospital Cash, Outpatient Surgery, Diagnostic Imaging, and Ambulance Benefit, how will I receive payment?
A check will be mailed directly to you, the member.

Is there a waiting period for all the available benefits?
Yes, upon initial enrollment, there is a 120 day waiting period for Employer Groups and 180 day waiting period for Individuals before claims are eligible for the benefit.

Can I get coverage for my dependents for the AD&D and TTD benefits?
No, dependent coverage is not available for the AD&D and TTD benefits.

What happens if I have a claim?
Call and notify us at 877.BEN.PLUS (877.236.7587) and we will send you a claim form for you to complete. Upon receipt, we will process it immediately.

Will this plan work in conjunction with my existing Medical Plan?
Yes, this plan will cover you as a supplement to your current Health Plan for out-of-pocket medical expenses.
